What Information does Amelia keep about you?
Amelia will take a full history at your initial consultation. She will ask you for general personal information as well as keeping documentation relating to your condition including referrals/ reports from your Referrer if applicable.
Amelia will keep up-to-date, accurate physiotherapy records which are essential for effective treatment. This is also a mandatory requirement of the Health and Care Professions Council (HCPC) and Chartered Society of Physiotherapy (CSP).

Who does she share information with?
You may have been referred by a Specialist Consultant or be under the care of other professionals. In order to work effectively and cohesively, Amelia may need to share information with these professionals.
In certain circumstances, Amelia may need to send reports to Insurance Companies to seek funding for further treatment. In these circumstances, Amelia will always ask for your consent to do this.
How does she protect your information?
Maintaining your confidentiality is treated with paramount importance by Amelia. She is also governed by the Codes of Professional Conduct of the HCPC and CSP
She is up-to-date with Information Governance Training as part of her NHS Practice and is aware and fulfils her responsibilities under the GDPR.
